Readiness venues

You can sign up for one of the readiness venues here for an HIV draw or vision screening

If you are in the NCR and would like an HIV order placed, please fill out this form and an order will be placed within 48 business hours) you can get it completed at any MTF in the NCR.

PHA

Please complete the online portion at your branch site, listed below. Once completed please reply to usumedicalreadiness@usuhs.edu stating you have completed the online portion and include your DOB. Once we have confirmation that your online portion has been completed and we have your DOB we will contact you if any further action or information is needed. If you do not hear back from me within 5 business days please do not hesitate to reply back to this chain for an update.

IMR Updates

Please provide us with the information you believe needs to be updated along with your DOB. We will look in your record and update what needs to be updated or we will let you know what needs to be done to get it updated. We will NOT respond letting you know it is completed. For Army, please allow up to 1 week for it to show up in your IMR. For Navy and Air Force, Please give us up to 5 business days to complete. We will let you know if we need further information.

Retirement Physicals

Part 1 needs to be completed at the WR readiness clinic. We cannot schedule your part 2 until part 1 is completed. Once part 1 is completed, send an email to usumedicalreadiness@usuhs.edu with the following information; 

-The type of physical needed (ex. Retirement physical, commissioning physical)

-Provider request (If you have one)

-Your DOB

-The time frame you are looking to book your appointment
